By participating in any online or in-person class or course offered by Rev6 or it’s affiliated teachers and programs, you hereby agree to the following:

Rev6 Participation – Release Of Liability

In consideration of participating in exercises and/or fitness related activities demonstrated by Revolution-in-Motion, Inc.(“Rev6”), I the undersigned (“Participant”) acknowledge and agree that:

• I have been advised to check my physical condition with my physician before engaging in exercise;
• The risk of injury from the activities involved in this program includes injury, permanent paralysis, and death;
• I willingly agree to comply with the stated and customary terms and conditions for participation. If I observe any hazard or risk of potential injury during my presence or participation, I will remove myself from participation and bring such to the attention of Rev6 immediately; and,
• I have been advised that I am allowed to have this Release of Liability form reviewed by a legal professional before signing it or choose to not participate in Rev6’s programs or events.

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless Rev6, their officers, agents, employees, affiliates, other participants, sponsors, advertisers, and owners and lessors of premises used for the activity, with respect to any and all injury, disability, death or loss or damage to person or property, whether arising from the negligence of Rev6 or any third party, to the fullest extent permitted by law. I have read this release of liability and assumption of risk agreement, understand its terms, and sign it freely and voluntarily without any inducement.

Permission for Photography

For valuable consideration received, I grant to Rev6, Inc. (“Photographer”) the absolute and irrevocable right and unrestricted permission concerning any photographs that he/she has taken or may take of me or in which I may be included with others, to use, reuse, publish, and republish the photographs in whole or in part, individually or in connection with other material, in any and all media now or hereafter known, including the internet, and for any purpose whatsoever, specifically including illustration, promotion, art, editorial, advertising, and trade, without restriction as to alteration; and to use my name in connection with any use if he/she so chooses. I release and discharge Photographer from any and all claims and demands that may arise out of or in connection with the use of the photographs, including without limitation any and all claims for libel or violation of any right of publicity or privacy. This authorization and release shall also inure to the benefit of the heirs, legal representatives, licensees, and assigns of Photographer, as well as the person(s) for whom he/she took the photographs. I am a legally competent adult and have the right to contract in my own name. I have read this document and fully understand its contents. This release shall be binding upon me and my heirs, legal representatives, and assigns.

However, a courtesy request of permission will be granted to members when considering using footage where a member is featured. We will not publish content where a member is featured without acknowledgment from the member.
