VIDEO SUBMISSION, PHOTOGRAPHY AND OTHER MEDIA: By submitting any video, photo, or other media to the website, the Submitter gives Rev6 the right to use, edit, or publish the submission without any compensation to Submitter. Submission of a video, photos or other media further constitutes the Submitter consents to irrevocably assign and transfer to Rev6 in perpetuity any and all rights, title and interest in the video, including without limitation, all copyrights and rights of publicity including the right to publicly display, perform, reproduce and distribute the video and/or excerpts on the Internet, in presentations, marketing materials and any other media Rev6 chooses and for any and all media now know or hereinafter devised. If required, Submitter will sign any documents necessary to transfer copyright and rights of publicity to Rev6 or another party which Rev6 designates. If a Submitter's video, photo, or other media is chosen to be posted on the Website, Submitter may have to certify in writing that his/her entry was original and produced by Submitter and no other person or entity holds rights to the video, photo, or other media Submitter is also responsible for obtaining permission to record, photograph, or include others in any media that is included in the submitted video, photo, or other media in writing. All video, photo, or other media submission materials become the property of Rev6 and will not be returned. Rev6 is not responsible for any change of email address of Submitters.
This authorization and release shall also inure to the benefit of the heirs, legal representatives, licensees, and assigns of Photographer, as well as the person(s) for whom he/she took the photographs. I am a legally competent adult and have the right to contract in my own name. I have read this document and fully understand its contents. This release shall be binding upon me and my heirs, legal representatives, and assigns.

OTHER: By participating in this training, you are agreeing to complete all of your own work. You are also agreeing not to misrepresent, share, distribute, or use the material contained in any way without first gaining written permission from Rev6 to do so.
The following terms apply to the Rev6 Foundations Intensive:
1. The term of the certification will be one year from the Certification date.
2. Certification of the Rev6 Foundations Course does not allow you to teach Rev6 in a class setting, or one on one with an individual or client.
3. Certification of the Rev6 Foundations Course, does not allow you to conduct workshops, or train other trainers or certify others in Rev6.
4. Exercises taught shall strictly adhere to the form and methods designed and approved by Dr. Heus.
5. Written and Audio Video materials given for instructional purposes are not to be reproduced without the written consent of Dr. Heus.

For valuable consideration received, I grant to Rev6​, Inc. ("Photographer") the absolute and irrevocable right and unrestricted permission concerning any photographs or video that he/she or enrollee in a course has taken or may take of me to use, reuse, publish, and republish the photographs or videos in whole or in part, individually or in connection with other material, in any and all media now or hereafter known, including the internet, for educational purposes without restriction as to alteration. I release and discharge Rev6 & Photographer from any and all claims and demands that may arise out of or in connection with the use of the photographs or video, including without limitation any and all claims for libel or violation of any right of publicity or privacy. This authorization and release shall also inure to the benefit of the heirs, legal representatives, licensees, and assigns of Photographer, as well as the person(s) for whom he/she took the photographs.
